To create your very own skins you need some type of pattern - that's the so called SkinKit (Default-Skins) that you can download »here (765 kB).
You have to use some image processing-tool like PaintShop Pro, Adobe Photoshop, Corel Photopaint or something similar. You copy the default-skin (skin-kit) you chose for your character to the skin-directory and rename it to charactername.tga.
Now open the skin and make your changes - maybe cut special regions of the skin, edit them and paste them back to the original skin-file.
To view the changes it is recommended to run Darkstone parallely and switch between your image processing-tool and Darkstone with ALT-TAB. You have to reload the save-game to make the changes in the skin viewable.
